It is the faculty member's responsibility to inform any affected department when a curriculum change is made. This form has been created to ensure faculty and departments across campus are communicating about curriculum changes.
Please attach this page to any curriculum form that encompasses changes which affect any other academic program on campus.
This form is a PDF and designed to be printed off and written/typed upon by any college that may be affected by a curriculum change.
If this form is not completed (when appropriate), the curriculum change will stop in the process until this requirement is met.
